EOBs are going paperless

Beginning August 1, 2024, SmartHealth members will no longer receive their Explanation of Benefits (EOB) in the mail. EOBs will now be available electronically through the Ascension One mobile app or here.  

How to access your EOB


Ascension One Mobile App

Step 1. Download the Ascension One Mobile App from the App Store or Google Play Store.

Step 2. Open the Ascension One app and create an account. Follow all registration steps. Please note that if you previously signed up for the SmartHealth app you can use the same log-in credentials. (Do not enter the alpha portion of the ID number).

Step 3. Click on profile in the bottom right corner and choose SmartHealth plan.

Step 4. Click on the Claims section and choose a claim you would like to view.

Step 5. Under Explanation of Benefits, click the tap to view link.


SmartHealth Website

Step 1. Visit the app page.

Step 2. Click Log In or Create Your Account. Follow all the registration steps. Please note that if you previously signed up for the SmartHealth app you can use the same log-in credentials. (Do not include the alpha portion of your ID).

Step 3. Click on account arrow in the upper right corner and choose SmartHealth.

Step 4. Click on the Claims section and choose a claim you would like to view.

Step 5. Under Explanation of Benefits, click the tap to view link.


Request paper copies

If you prefer to receive your Explanation of Benefits in the mail, you need to login here.

  • Click on the profile button in the top right corner
  • Choose "Paper"
  • Click Update
  • Please note: If you already filled out the Google Form you do not need to fill this out
  • As a reminder, this will not take effect until August 1, 2024.

If you have questions or need additional assistance, contact customer service at 888-492-6811, Monday through Friday, 8:00 a.m. to 7:00 p.m. EST.  Please note customer service cannot change the EOB status. Members must fill out the form.
