What is care management?
Care management is a collaborative process to assess, coordinate, monitor and evaluate services and options to meet your healthcare needs and goals. By working with your doctors, our care managers can help you manage your medical conditions more effectively. Care managers can also provide you with education, resources and the encouragement you need to support your healthcare journey. Our national care management team is made up of registered nurses, social workers and wellness coaches to help you make informed choices about your care.
