Ascension’s National Care Management team launches a new Enhanced Disease Management (EDM) program for diabetes this month, July 2021.
Designed to help patients self-manage diabetes, the EDM program will provide personalized, coordinated care in partnership with doctors and a team of specialists, including registered nurses and certified diabetes educators.

The team will support and provide education on:
- Medications.
- Self-blood glucose monitoring.
- Nutrition.
- Exercise.
- Weight management.
- Scheduling annual visits; and more.
If you are already receiving care for diabetes, the National Care Management team will be calling to offer you the option to participate in EDM for diabetes.

The EDM program comes at no extra cost and it is part of your SmartHealth benefits.
The EDM program for diabetes is the first of several EDM initiatives Ascension is launching to help SmartHealth members keep healthy and chronic conditions under control.
